By registering for and participating in the Copa Athletic 1v1 Series, I acknowledge and agree to the following:
1. Assumption of Risk
I understand that participation in soccer and related athletic activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, and other risks that may result in serious injury, illness, or death. I voluntarily assume all risks associated with participation in the Copa Athletic 1v1 Series.
2. Waiver and Release of Liability
In consideration for being allowed to participate, I, on behalf of myself and/or my child, hereby release, waive, and discharge Copa Athletic, Copa Athletic 1v1 Series, Provo Indoor Soccer, their owners, operators, directors, officers, employees, coaches, volunteers, agents, affiliates, and representatives from any and all liability, claims, demands, or causes of action that may arise from participation in this event, including but not limited to injuries, property damage, or loss resulting from participation.
3. Facility Acknowledgment
I understand that the event is being held at Provo Indoor Soccer and agree that Provo Indoor Soccer and its staff are not responsible for any injuries, accidents, or damages that may occur during participation in the event or while present at the facility.
4. Medical Treatment Authorization
In the event of injury or medical emergency, I authorize event staff to obtain emergency medical treatment for the participant if necessary. I understand that I am responsible for any medical costs incurred.
5. Participant Responsibility
I agree that the participant will follow all event rules, facility rules, and instructions given by event staff. Failure to do so may result in removal from the event without refund.
6. Photo and Media Release
I grant permission to Copa Athletic and Provo Indoor Soccer to use photographs or video footage of the participant taken during the event for promotional, marketing, and social media purposes without compensation.
7. Parent/Guardian Agreement (For Participants Under 18)
If the participant is under 18 years of age, I certify that I am the parent or legal guardian and that I agree to this waiver on behalf of the minor participant.
