ACKNOWLEDGEMENT OF RISK AND RELEASE OF LIABILITY

I, the undersigned, acknowledge and agree to the following:

I understand that participation in recreational soccer involves inherent risks, including but not limited to sprains, collisions, falls, and other physical injuries.

I certify that the participant is in good health and able to participate in athletic activities.

I voluntarily assume full responsibility for any and all risks of injury, illness, or damages that may occur as a result of participation in Utah Athletic's Shooting and Finishing Clinic.

I understand that these sessions are not supervised by certified medical staff.

I acknowledge that Utah Athletic, their staff, volunteers, and affiliates are not responsible or liable for any injuries, accidents, losses, or damages incurred before, during, or after the event.

I agree to release, discharge, and hold harmless Utah Athletic from any and all claims, demands, or causes of action arising out of or in connection with participation in open play activities.
